Yes. Sometimes it was the client's fault, sometimes my fault, sometimes the fault of management.
I did a job a couple years back that was a total production wreck. It was a folder with a number of loose multi-panel brochures within. Never got copy for the last brochure, and I was so mind-bogglingly busy (I think I had 12 live projects at the time), that it escaped my notice as well as the clients. They printed the last brochure in the piece with "HEADLINE GOES HERE" in 42 pt Bauer Bodoni. Part my fault, part client fault. Thought I was going to lose my job over it. Ironically, I lost my job two years later, completely unrelated to that job or any other.
