Just centre the cutter with the spine in the middle of your file, you'll probably have more space on one side but who cares. On a new layer arrange your aw accordingly. Then duplicate the page, lose the aw on that page and flip the whole cutter keeping it centred. Arrange aw on the reverse.
Save out a PDF with page one as a visual, aw on, cutter on. Page 2, just the cutter. Page 3 just the aw. Page 4 just the reverse aw.
This has always worked fine for me.
