i've just setup the cutter guide as a separate layer so the design correlates and works with it, suitable bleeds - make sure to do a scale mockup just to double check. I've always let the printer sort out the rest I suppose if you are unsure ask the printer for advice on how they prefer the files to be setup. hope this helps
