Welcome to the wacky world of project management!
Two easy ways to deal with this without losing your mind:
1: If you're the one planning it, assume you will have both delays and scope creep for every single milestone. As you come up to one, think 3 days (on average) of delays and changes past that and plan for it.
2: If you've got high risk or high maintenance clients, you should be adding a minimum of 10% contingency across your project budget. The more high-risk, the less transparent you should be.
Clients should learn that if they keep screwing around, it's going to cost them.
