i actually have to do something very similar for myself within the next few days.. if i make any progress before tues and you still haven't found anything i'll let you know, but i can't make any promises.
to clarify:
you want it to be able to submit this file along with a form.. but isn't this being put on a CD?
if so, how can it be self-contained and where is the url you'll be submitting to?
