can't really think of any big ones for work. With most of my stuff being web based any fuck ups can be changed/covered up pretty easy. I'm enormously paranoid about print jobs going away with mistakes so I check and recheck everything like I've got rampant OCD.
I did leave out the venue on a club flyer once and the promoter had to hand stamp all the flyers, but that was just a wee homer and didn't amount to much hassle
