I think this kind of job will require you to visit a print shop in person.
it shouldn't be a problem if its one color.
I've done something like that (a lot uglier) many years ago in 3 colors for a soap manufacturer, the final product looked nothing like what you posted above but I'm sure is a similar process.It was a roll of that the machine will print, then fold and seal.
