  top three things (because there are hundreds...)my boss does that makes me die a little bit inside:
  -when he wants his creative 'team' to know that he's working super hard, he'll rub his hands together really loudly- and exhale deeply, followed by more hand rubbing
  -he calls people on the phone 'guy'. even if he doesn't know the person that well, he wants to appear sociable and hip (for example: "well great tom- i'll get the specs over to you asap. yep. okay guy, great!")
  -he walks in the office after using the restroom, and says to all of us designers: "hey peeps, anything goin' on?...how we doin'??"
