Ok, this one is bad. Real bad. Working on an annual report a few years ago, was very close to sign off and I deleted the folder. Deleted everything. So we had two days to get a full version together while the client made corrections to the last hard copy we had sent over. We got it all done but in my haste to get it finished, I used old artwork for the cover, which had the wrong year on it.
So a couple of weeks after it went out we had to get all the copies back in, reprint the cover and remove the old ones and reattach. I nearly threw up when I had discovered I had deleted the files and that was me back on the cigarettes.
