The manufacturing company I was working for at the time didn't have a dev. server so I worked right on the live e-commerce site and, as you'd imagine, there were a number of times I broke it. Most of the time I noticed right away and fixed/undid the problem but one time the shopping cart was unusable for hours. Oops.
