    Not got any direct link of use to you - but I would think, 'yes', it is a usable approach, assuming that it's all well laid-out. I know that I have certainly done it over the years.

    imho, the principle consideration is how the columned form plays with the 'below the line' break in your page - I'd ensure, for instance, that the column height didn't extend beyond a visible page height - if it needed to, I'd consider adding more columns.

    The flip side is to whittle down the questionnaire options to as few as possible - too much choice is usually not a good thing.

    Try and dig out a printed questionnaire from a big company - someone who'll clearly have spent a lot on it. They'll have done all the expensive, clever consumer interaction analysis - it'll just be up to you to ensure that works on-screen.
