  I am in exactly the same situation friend, I am handing my notice in tomorrow :/
  I have been here for over seven years and I have been through shit loads with my boss and it is a very small company as well, it's hard and I'm scared but I have to do this which is keeping me calm.
  I simply put a one sentence note saying

  dear
  I am writting to give you notice that I wish to resign my post with effect from October 25th.
  Kind Regards
