    I think 'vorm' would be better translated to 'shape' than to 'form', although the verb 'vormen' could be translated to either 'to form' or to 'to give shape'

    - external appearance (shape)
    as in: that sculpture has a nice shape

    - physical stamina (shape)
    as in: she is in good shape

    - behaviour / manners (form)
    as in: good form, old chap!

    - verb / to form
    as in: I'm forming an opinion

    in Dutch the traditional translation for designer is 'vormgever' wich translates to 'formgiver'
