I think the professional D&AD awards are a bit of a clique thing. It's always the same companies, but then, the work in the annual is consistently good.
In terms of paying for entry, D&AD is a non-profit organisation. All the money they make goes into design education. They get the best designers to be judges at their awards – creative directors from all around the world, sagmeister etc. It's only fair to have the most informed judges, these people can't take a few days off work for nothing, plus there's all the production and publishing costs, and they sponsored as well.
Overall I think they're a good idea, they shouldn't drive all work and be the 'be all and end all' but if they help motivate people to produce excellent creative work, then why not.
