This can be done in photoshop fairly easily.
1) Open your photo and change the mode to 'Index Color' (Image->Mode->Index Color)
2) An options box comes up. Just change it to 'Local Perceptual' and the number of colors to '256' and Dither to 'None' Click 'OK'
3) Bring up the table by going to 'Image->Mode->Color Table' Save and you're done.
Easy as Pie.
