I think this could be a bad idea MX.
Clients like to feel loose and free with their projects but ultimatly feel that it is entirely theirs, so a form to fill out with changes they want to make may really piss them off or make them take the next job elsewhaere it seems a bit anal and somewhat stale, let them put things in emails and try to deter them from just telling you things over the phone.
