    simple artwork:
    convert an image to bitmap (image > mode > bitmap). the file will be 1-bit, meaning the file is considered 'on' or 'off' to any other app. the 'on' designates artwork and the 'off' means zippo and therefore transparent.

    not simple artwork:
    you need to create a clipping path (deep-etch, clear cut, whatever you want to call it) and save as an EPS. the EPS file contains 2 pieces, a preview graphic (usually lower res when placed than a tiff but prints perfectly) and file info which includes the clipping path info.

    psd: won't work. will work onscreen usually and will work with some new and advanced setups (printers / rips etc) but is still a dangerous way to go UNLESS you're supplying a PDF and you're using CS2 as CS2 handles the transparency well whereas anything previous seems to bend you over the couch for laughs.
