Depends on the fidelity you want - you'll probably be saving a copy in compressed format anyway, so any cheapo capture card will work fine.
As for software, the free choice is Windows Media Encoder, the expensive but more standards-compliant choice is RealProducer/RealServer (now called Helix), or you could investifate broadcasting in quicktime, which I believe can be done using opensource products.
