i use a combination ot the path toool/bezier pen and masking brushes... I have a set of brushes specifically for masking and drop shadows... you can also dupe channels and nuke what you don't need i usually dupe channels for fringy hair... when my company gets really loaded with a lot of masking work we just upload our files to india and we get multichannel psd's back containing the masks they can usually turn around 100 images and 24 hours and they mask everything flesh,dresses, jewelery... we used to send them to china but they wanted too much so now we send to india... i think it works out to 5 bucks a mask but we have a bulk contract... It's sad really just one more step closer to outsourcing....
