I use the 12x18 and have for about 9 years or so. Worked at a place that ran ads and we all got one. Ever since that experience working there I havent gone back to a mouse. Who needs to design with a brick. Also writers cramp is a bit better than the wrist. Try that one as a test (if thats at all possible) and youll see why. The keyboard becomes secondary which messes up your back a bit swinging around but overall you only use your keyboard as a suppliment to your designing. Main reason why 12 x 18 is the best is its relative to the screen. When you use a small one your slight swoop throws the arrow on the screen flying. With the large it goes where you go accurately.
Downside is the cash.
