What about a medical theme. You could have the book look like a doctor’s bag.
If the paper has a really clean look then use images of modern medical facilities and if it is old or natural looking use some 19 century English medical imagery. Perhaps a witch doctor twist would be a nice contrast within this theme.
Lots you can do with this and I haven’t seen it done anywhere before. You can throw in medical information/beliefs from each era.
