i haven't read this whole thread cos i've got to go to a meeting, but my da works with this subject.
a) yes, it is as real as alcoholism or drug addiction.
b) i reckon it's probably equally as prevalent as the above, but, due to the stigma attached, not recognised as such.
c) you can bet your sweet bippy that, same as any addiction, it's ruined marriages, careers and lives.
