i always put them on when i do the package. We had a software that created the vector art for you after you fed in the numbers. Sizes varied depending on size of the package for the most part, but you can't let it get too big.
You also don't *have* to use the full height of the barcode. I've made some that looked squat because i shortened the length of the lines (though the vertical and horizontal scale remained at 100% each).
