versions: psd's v groups v..

one file, no v2, v3, v4 etc, no multiple comps in one file. no hidden layers or groups
layervault keeps every save as a revision. if the client liked the one with rounded corners I just revert to that revision

You could also just use Git or SVN. They both have no problem with PSDs (or any file type) and are free.

I typically do separate files, versioned by date and version number. I don't necessary want my PSDs cluttered with old stuff that didn't make the cut, which would be the case if I only did a single grouped PSD.

multiple file versions until a direction is signed off on.
i use layer comps only for final files that need slight variations in text or promos or how a shot is cropped. they are incredibly useful.
