just because your client has a 386 with a 13", 256 color monitor it does NOT mean you should design the site within their specs.
We recently had a client request 800x600 because that's what computer the boss had. They never got a call back because we all knew that was a red flag for a pain in the ass client.
Either do you best to inform them with logic, reason and facts, or dump them if they are firm in their request. Many headaches are ahead iwth this client dude.

Simple solution: Ask them to send you some example sites that match their requests :)

Ask them if their monitor supports VGA or SVGA.
Ask them if their 386 is an SX or the sweet DX with the math coprocessor.
Ask them if they splurged for the 4 MB of Ram.
If its the DX, Doom must run pretty sweet on their rig. Prolly have a Soundblaster 16 to crank the midi music too.

Make a mock, and have a Zoolander lookalike come along as a heckler. Midway through demonstrating have the heckler say:
'What is this, a website for ants.' Have him throw the computer on the floor before claiming 'This website needs to be at least 3 times this size!'

Genuine suggestion, show them a mock of a website at 640X480 on various common browser sizes. They should see how retarded their request is on a full 1080p monitor.
Explain that the best way to go is to adhere to, say a 960 width site with scrollability, possibly with a tweaked stylesheet to kick in for mobile.
I think that either they are idiots who couild do with being educated, or someone in the department has said 'well what about an iPad, we have to constrain the full capabilities of our main showpiece site to fit that dimensions' and then neglected to realise iPads are higher res than that.
