"I hear the engines revving. The lights are glowing. I'm hearing the alarms. I mean, they're at a constant state now. It doesn't stop. But even that's starting to get drowned out by the sound of the engine increasing in speed. And my lights get so incredibly bright that they physically explode. I'm pushing my way back from the desk when my computer monitor exploded," Williams told Pelley.
Pelley jumped nearly 100 feet into the dark burning waters below to save himself.

Seems to be squarely BP's fault according to Pelley, by rushing the temporary 'cap-off' of the well, and a faulty Blow Out Preventor they knew was bad, but were almost ready to cap the well and pull the rig away.

I've been thinking... what they're doing now, cutting the pipe to put a cap on it (in which the saw got stuck today - http://news.yahoo.com/s/ap/us_gu…) - what if when they get the cap on the pipe blows out in another location? It's just been setback after setback. I'd figure that the pressure of the oil coming out of the seabed can't just be stopped by capping the broken pipe.

22-mile long, underwater invisible plum detected by scientists.
Florida State University scientist Ian MacDonald, in testimony before Congress on Thursday, said the gas and oil "imprint of the BP discharge will be detectable in the marine environment for the rest of my life."
The underwater oil was measured close to BP's blown-out well, which is about 40 miles off the Louisiana coast. The plume started three miles from the well and extended more than 20 miles to the southwest. The oil droplets are odorless and too small to be seen by the human eye. If you swam through the plume, you wouldn't notice it. "There's no visible evidence of oil in the samples; they look like clear water," study chief author Richard Camilli said.

BP: Oil containment efforts resume after ship fire
HOUSTON – A drill ship resumed siphoning off oil gushing from a blown-out well in the Gulf of Mexico on Tuesday after a bolt of lightning struck the vessel and ignited a fire that halted containment efforts, the company said.
