tripod, bulb exposure,

Why try to do it in camera digitally though? Either do it on film as a genuine double exposure, or just shoot a few handheld shots and comp them in photoshop. Its not like you're reaching for the 'chrome lizard scales' filter is it. Photoshop is, underneath all the fancy bollocks, a simple virtual darkroom.

tripod, open shutter, dark room, 2 flashes?

Yeah. I was thinking about using flash and moving a model with open shutter.

Unless this is an art project, just do it in PS — you'll have more control in the end.

a lot of nikon dslrs do multiple exposure and in-camera compositing of multiple shots

Long exposure, dark room, pop your off camera flash twice.

on my nikon d80, I select multiple exposure, take 2 shots and ba da bing.
Job done.
But I'd be more likely to either do it with a flash to get a cool effect, or just take two pics and shop them
