plenty of designers get royalties. sell a wordpress template, sell an icon set, sell a photo or illustration -> royalties.
royalties in consulting work however are generally only attached to a company that can't pay full-rate and therefore agrees to an equity swap or ongoing payment. These are mostly bad customers with shakey finances that want this sort of arrangement, and best avoided unless you really believe in them, and can see some potential for their success.
