It may be the host, it may be the web site code, or both. If it's an out-of-the-box CMS like Drupal or WordPress the best you can do is make sure it's running the latest version. If it's custom code then you may need to look for vulnerabilities. If it's a static HTML site then it's most likely the host, but also something as simple as bad control panel or FTP passwords.
