Hijacked Index Page

I've come across this more than once...
... but it's not my fault. (grr)
There are some nasty little scripts that get embedded on some index pages from time to time...
... I just found one on a previous client's website. They'd go to launch their site and then get a malware warning type of deal...
Now - is this a HOST issue? I'd think it would be - their servers are being infiltrated... yes?
Well - the host tried to put it off as a design / code error... ... WELL NO SHIT... but it was YOUR SERVER that allowed whoever in to add that line of script at the very very bottom of the index page... right??
Is there a way to avoid this issue? Is this a hosting problem?
I've already switched out the hacked index for the new one... but that's not really the point... ... I don't want this happening.

It may be the host, it may be the web site code, or both. If it's an out-of-the-box CMS like Drupal or WordPress the best you can do is make sure it's running the latest version. If it's custom code then you may need to look for vulnerabilities. If it's a static HTML site then it's most likely the host, but also something as simple as bad control panel or FTP passwords.
