Concrete5 Job Application Add-On

I'm trying to build an online job application "form" for a Concrete5 website and I'm stuck. I've looked for suitable plug-ins/add-ons to purchase and I can't find anything that looks like it will do the trick. The user will be able to fill in fields, pull from drop-down menus, etc. and the submissions will be sent to a client (HR) email address.
Yes, I know there are tons of options for other open source cms systems, but it has to be Concrete5.
Also, I know there's a vibrant Concrete5 community that exists for such requests, but I haven't had any luck there. I thought I'd reach out to the design community - where people actually care what things look like... and understand the nuances of agency/client relationships. Any and all suggestions are greatly appreciated!

What version of C5?
You can easily do that without any addons, the core Form block works just fine, you can also use Formidable (https://www.concrete5.org/marke... if you need additional functionality.

Thanks ESKEMA! We are looking at Formidable and Formify right now and trying to decide (or add anything else in the mix). Here's the 411 on our versions:

If you're on 8.2.1, just create a new Express Entity with all the options as attributes, and add it to a page. No need for addons unless you want additional functionality.
What would that form look like? What questions / type of questions do you need?
