anyway, fast looking into your code, what are the br tags for?
and maybe you should specify the type for the mp4 video too.
and why is 1 url relative, the other absolute? (they don't seem to point to the same directory)
clear the code maybe and maybe the cage of chrome?

it may appears the problem is on ajax call... when you do the ajax, it is retrivied several javascript errors
- yeah it works fine - just when you call via ajax - not sure how to fix it though
