Like someone else said, its just effort. Photoshop, Illustrator, its not hard to see how theyre done, how they could be done, but actually doing it is a different story. Each would take a lot of time.
And of course thats just the execution. You first need the idea, and how theyre going to be designed.

Probably a skilled illustrator that starts with hand drawing roughs and then translates that into illustrator. Its a certain style that takes many hours of drawing and particular taste to achieve.

I'd say 1st is made with illustrator blender tool. but I don't see any particular tool/trick used on the other ones. maybe some custom brushes used for the lace shadow?
it's more about talent and work than anything else.

Alex's work is astounding, but just like any artist, great work begins with a thought process - it's nice to include his sketching in the collection - I've often found an artist's sketching more interesting that the final work in some ways.
The process is a journey, the finality of a piece is almost sad, but with art, as in life, there is an end.
