usually their designs are hand made and not too sourced so there is cost to the designer there, then barney's overhead, then barney's markup.
look at some of the made in usa jeans like roy denim, acne, etc all selling very expensively, you're buying a boutique item for a premium markup when you buy these items through retailers instead of direct from the manufacturers

I made this as an exercise:
Materials + tax + shipping + some ther fees (cards/paypal) = around $30
If I sell it, common rule I'd sell it for $60. Then it'd be resell by retailers around $120 <-- I think this is too pricey. If I do big run then I could almost cut it by 30-40%.
Now on the shirt, judging on those pics alone, it doesn't look like a $400++ shirts. But I have sewn a custom shirt myself, using a fine shirting cotton by Scabal @ $65/metres. Based on that, then it could be around that price on stores. But mine looked way better than that :).
