Designer, actual talent
Creative, knows how to type keywords into YouTube- This is almost the dictionary definition. Creative occasionally misspells keywords.

Designer - has been failing for long enough to know what works and gets there quickly, executes it well, is frustrated by people who know less and are in a position to direct him/her (art directors, clients, CD and in some places marketing 'professionals')
Creative - Is valued by clients enough to be listened to because they don't understand what it is he/she does/knows, earns more, is basically a designer - but my not be, in some cases has no skills at all.

"Creative" is generic – "Designer" is specific.

designer is far from specific...
you need more descriptors to get the point across.
graphic
web
3d
sound
lighting
stage
etc etc etc

I've generally found that anyone who calls themself a 'creative' when you ask what their job is, is
1) not creative
2) a wankercheers

"Creatives" are the managerial class.
"Designers" are the working class.
"Creatives" give "direction"(tell what to do) to the "designers".
"Creatives" are valued for the personalities and social skills.
"Designers" are valued for their ability to follow directions and skills in whatever software is currently popular.
