For employment at an agency or client?
Either way I put it as "upon request".
And if requested by a possible client I try to match them with either people in the same industry or people that could otherwise benefit the client. If they seem nice.

In the UK it's considered good CV etiquette to add 'References available on request' - and then speak to your References and give them a heads up to let them know they might be receiving a call or email about your skills, responsibilities, professionalism etc.
- And I would not give more than 3.MSL

I think references for your two previous jobs is more than enough.
In my experience, nobody really cares to much about them if you do a good interview.

It probably only depends on the job sector.
In advertising I don't think many people are worried about the references, more about what you can do.
Most people in the industry are slightly talented, moronic, ruthless cunts. So they are probably aware that you are one as well.
But you should still be able to do the job you are applying for.

Do references really matter anymore? I mean now, legally, you are not really allowed to defame anyone, you're just supposed to say that you can't comment if you have negative things to say.

I drive a porsche, also when I hire people, I don't really check people's references. I really look at their portfolio, past experience and their personalities.

Just put that you have References available upon request. Then have at least 2 ready during the interview. Don't give them out unless they ask.

Make sure to put a high school athletics coach as a reference. Nothing makes a potential employer more assured of you than knowing how great you were at the 40 yard dash.

Per my comment above, a nice a 'legal' reference interview is terribly boring. You're supposed to stick to basic yes or no questions like "would you consider them eligible for rehire."

Always?
