    you're video doesn't exist,
    try verifying if it is inside the right folder.

    <config>
    <param name="flv" value="cio.flv"/>
    <param name="width" value="220"/>
    <param name="height" value="165"/>
    <param name="bgcolor" value="cccc99"/>
    <param name="playercolor" value="111111"/>
    <param name="bgcolor1" value="000000"/>
    <param name="bgcolor2" value="000000"/>
    <param name="buttoncolor" value="dddddd"/>
    <param name="buttonovercolor" value="f9bf37"/>
    <param name="slidercolor1" value="dddddd"/>
    <param name="slidercolor2" value="cccccc"/>
    <param name="sliderovercolor" value="f9bf37"/>
    <param name="showstop" value="1"/>
    <param name="showvolume" value="1"/>
    <param name="showtime" value="1"/>
    <param name="srt" value="1"/>
    <param name="startimage" value="video_img1.jpg"/>

    Don't forget, if your html and main swf are in different places, your path must be relative to the html and not the main swf. It's stupid, but it trips me up every once in a while after more than 10 years flashing.

    make sure your server recognizes .flv file types.
