How would I include an image in a paragraph style so for example I had an "answer" portion and I wanted an image to appear above it each time I applied that style?
Also how do I set it up to ignore the rule I have placed about text when it is at the top of a column and if I can't do that how do I override that one instance without creating a new style?

I don't think that's possible... unless it's the type of image that you could vectorise and add to the font.

Maybe as some sort of background element to the style/text box? Only a guess, sorry, I don't use ID much.

You could most probably set up a script to do something like this. Never done anything like this myself though.
You can override the top of column text just by selecting it and restyling it the way you want.

Object > Anchored object > Insert for the image part. I don't think you can apply that to a paragraph or character style, but if you copy and paste at least it will stay inline with your copy as you edit it.
You could use nested styles to create a first paragraph without a rule and all subsequent paragraphs to have a rule... or a character style you apply to either the top paragraph to remove a rule, or the rest to apply a rule. If you're linking your text boxes there's no way to tell InDesign to detect a paragraph that's at the top of a column.
I know you don't want to use another style, but the only other way is to manually remove the rule by selecting said paragraph and using Cmd+Option+J (or the paragraph panel flyout menu) to open the paragraph rules and turn it off there.

I think turnerworks is right, as far as I know you can't apply an anchored object to a paragraph or character style as it's basically a special text character. You're probably gonna have to add the image to your answer sections manually :(
You can set up object styles to control the appearance of the anchored object/image, but inserting the image is probably gonna have to be done by you and your fingers.
That nested styles shit is a little bit over my head (though I am slightly drunk) so I won't even try to help with the rules thing.
