ftp distribution

I manage the electronic distribution of an agency's pdf's to publication's ftp sites which is simple. The mundane part is having to fax a form to each publication containing file name, image and information to be completed and faxed back. I currently have an Indesign file for each publication and place an image and file name. I am thinking there must be a more automated way of doing this? Anyone doing similar?

why fax? why not just a pdf that they can fill out and then sends info to a pre-specified email?

Some are small town publications that in some cases that don't seem to be able to work with a pdf with fields and comments!

yeah, a web form with an image and text field. the free version of pdflib will let you place an image and some text into an existing pdf doc and save it out as a new pdf using php, perl, python, etc.. i made something similar using php to generate temporary visitor IDs inserting name/photo, I can send you the code later if you'd like.
