oh, just got back, but we are not trying to collect any information actually, just an email gimmick i guess...

Like Josh says, you should do this online - the html mail is the lure to the hook that is your form.

they want it to appear as though they are selecting their answers within the email itself... then there is a button after the options that is linked to a page they want them to go to...
