Web Form vs Function

The form is probably one ugly monster to design. It's boring fill out much like an IRS form. But it's also an art and lots of information thinking is involved within its construction.
Are there any forms out there that deserve the right to call itself creative? The best I've seen has to be apple's search form off its homepage. (http://www.apple.com). It was quite clever and would like to see more, but it uses only one text field.

i've noticed alot of generic forms..... and wonderful forms they are... but are there no personalized web forms out there. i'm even thinking that perhaps some flash forms would be a good thing to look at.

although i missed kona's link post. the forms on there are actually quite interesting. still i'm not opposed to the generic direction, but still you'd think there would be some swiss designed forms out there
