i mean, to me it looks like a telecom ceo card. doesn't fit the creative person too well ihmo - but it's definitely business-like.

underscores? cool, but why the trailing one every time? Runs my eyes off.
4 or 5

Looks a little like a calling card.
But yeh, 4 or 5.

4...
if you take out the circles. 4 works 'cause it's clean.
3 as a secondary choice, though the color bars need to go. scrap the circles. they add too much unnecessary-ness. add more leading. i'd also love to see a version like 3 with text that takes up the majority of the space to highlight the type.

No underscores. 2002 is over. No background crap. It serves no purpose. No screened colors, if that's what your secondary lighter blue is. Be careful if you knock type out of a darker background, it could disappear if they do a double hit of the blue.
I'd look into synthetic materials, seems like it would be appropriate for this one (Yupo, Gilbert plastics etc.)

I like #1 and #2 but without the '_' - not needed
or maybe delete itfrom theend of NAME, TITLE, EM - not needed
# 4-5 to traditional for me, but they are a lil more serious >:-|
