We always do custom. We reuse a lot of components but it's always custom. These are high end clients with a budget that are looking to stand out with something original. Pre-built themes are usually extremely bloated and slow everything down, but they do have have their place in the market. I've used them in the past for freelance with clients that don't have much of a budget and just want something up. Honestly the pre-build themes can sometimes be more of a pain customizing than just building it from scratch.
