I've done this for a client before, huge pain in the ass.
1. Backup site.
2. Create copy of site on different server.
3. Use a plugin to detect unused media files and delete them. I've used https://meowapps.com/media-clean… before.
4. If everything looks okay, then restore this site to the live server. If they're using a plugin like VisualComposer you're probably fucked.
Going forward, you can install a plugin that will automatically resize the original images to a reasonable size, and optimize them. For example, when the client uploads a 5MB jpg from their phone, it resizes it to 1440px wide and deletes the 5MB file, leaving you with a reasonable 200 KB file instead.
