Extremadura is a flat featureless expanse. Not sure what would be attractive there.
Granada & Bilbao would be my two recommendations (awkwardly, given they're at opposite ends of the country), and although I've not myself been, everyone who's ever been recommends Barcelona.
My folks live in Andalucia so it's a place close to my heart and, where food's concerned, belly. Tarifa's chilled and has a vaguely Oz-like scene, beautiful beaches sea sports and a stunning view of Africa, across the straits.
Personally I'd not bother with Madrid, but I hear it's come on leaps and bounds in recent years.
