On my experience it's always hourly. and in regards of rates,
I think it depends on:
a) The Agency
b) The Designer
c) Ability to negotiate
It does not depend on:
a) A standard rate
b) Designer experience
A great factor in the negotiation:
a) How much the agency wants you
b) How many projects are you working on currently
With that being said, I've seen in go anywhere from $20 to $150 per hour.
