I did exactly this a month or two ago and it was pretty easy, actually. The tricky part was getting the client to properly set up their merchant account but once that process was complete it's just a matter of generating and plugging in some API information from PayPal. I wasn't able to get the sandbox mode working but you can buy something cheap then issue a refund. Once it's all working, it's quite seamless since the customer never leaves the site.
