Cefalu is nice, you've got it on the photo. Take a boat trip to the Eoli Islands from there.
In Palermo, check out the interior of Hotel Ballaro, formerly Orientale. Walk up the stairs, it's incredible. Stay there if you haven't yet.
In Trapani, take a funicular up the mountain to Erice. If you have the time, take a ferry to the island of Favignana, and stay there for a day or three.
Get on top of mount Etna if you can.

Nice! Love Sicily and would like to visit many more than 2 times that I have.
Palermo is great - make sure to check out the catacombs, if that's your thing. Ragusa Ibla is beautiful. Syracuse – nice. Taormina was cool if not a bit posh/touristy. Enjoyed Catania. Messina I wouldn't bother with (a bit too industrial). Mt. Etna, like raf said.

Been many times (my wife is half-Sicilian) and it's a wonderful place. Etna is breathtaking, Catania and Palermo are both interesting and beautiful. The whole north coast is lovely (including Cefalu which you've pictured) - mostly rocky beaches there.
The simpler local food and wine is often the best, just great ingredients done really well. Seafood is great and gelato is a must.

Our youngest daugther is going there in Sep. for Erasmus. I love the scenery, and the weather... I'm sure we'll make a stop by ;)

In a surprising turn of events, I'm going to Palermo this Easter weekend.
I've been to Sicily a few times, usually flew to Trapani and took a half an hour ferry from there to Favignana, a small island in the Egadi archipelago. I've been to Palermo and Cefalù twice but I just realised it was 16 years ago.
Where do I go that I haven't been? I haven't really explored the coast from San Vito Lo Capo through Cefalù. What's in Palermo that I missed?
