How does one determine the value of a site, and then how does one go about selling said site?
Are there companies set up to handle this type of thing?

selling amazon.com is a different venture than qbn.com or an empty longrandomsitename.com
If you're looking to sell for growth / expansion, seek out investors, they'll quickly tell you what you're worth, whether it be low or high and whether you're worth working with.
If you're just looking to liquidate, I'd say assess the operating costs, any physical holdings, such as equipment, materials, etc. compared to monthly expenditures.
If it's just a site with traffic and a history, you're really only selling a brand and past content on some server, I'm sure the answer this as much as someone will give you.
Do you have an email list? Would you ever sell it to a third party? this might be that moment you fuck over all your subscribers letting that switch from your hands to theirs with the sale of the brand / content.

FWIW, I tried selling a site on flippa a year or two ago. It sucked. Totally not worth it.
Of course depends on the site/app/content, but it would be best to seek out someone who sees value in what you've created - then try to sell to them.
I've never done it, so that's the best advice I can give.

Do you intend to sell your blog?
I suppose you tried various monetisation options that didn't turn out satisfactory, but I think it has solid earning potential.
Have you considered changing the formula, transitioning the blog into something closer to an online magazine?
Selling a niche blog is tricky. I would start with determining who might be financially interested in owning, or sponsoring one, then approaching them.
A great example of a blog gone semi-online-magazine is http://www.carryology.com
