Why so black & white? This puts a lot of blame on the designer (hourly? Wrong!). In most cases, i.e. reality, I believe client stalling/revising is sadly the norm, and something best handled/eased by hourly billing. A hybrid, or clause for hours on top of a project fee, would imho be a good weighted option.

None of that "hours? bad designer" crap. Be flexible.

bit of a sensationalist title - esp when it suggests itemised costing based on hourly rate as solution...

which seems pretty fucking obvious... some of my clients insist on billing by the hour + no other way for maintenance contracts.
